Effective care planning is central to good practice in children’s social care. A good plan should provide much more than a record of identified needs or a list of tasks for professionals to complete. It should create a clear connection between what we know about a child or young person, what they need, what we are trying to achieve and how support will contribute to meaningful change.

Course Content
The course begins by exploring the relationship between assessment and planning. Participants will consider how information about strengths, needs, risks, experiences, relationships, identity and aspirations can be analysed and used to establish meaningful priorities rather than simply transferred from an assessment into a plan.
A key focus will be the difference between needs, outcomes, actions and interventions. Participants will explore how poorly defined outcomes can lead to plans that describe activity without explaining what should change for the child. The course will consider how to develop outcomes that are meaningful, realistic, measurable and understandable to the child or young person.
The course will examine the role of children and young people in planning their own care and support, including how professionals can ensure that wishes, feelings, aspirations and individual perspectives influence both the plan and the way support is delivered.
Participants will also consider how plans should address risk and safeguarding without becoming dominated by risk management. Attention will be given to strengths, relationships, education, health and wellbeing, identity, independence, family and social connections, and other areas relevant to the individual child.
The course will explore the relationship between the plan and everyday interventions. Participants will consider how key work, direct work, routines, activities, professional support and multi-agency interventions should connect back to identified needs and agreed outcomes.
Finally, the course will examine review and evaluation. Participants will explore how to determine whether interventions are effective, recognise evidence of progress, respond when outcomes are not being achieved and ensure plans remain current as circumstances and needs change.
